If you can't call the surgery during our normal opening hours, you can contact us online from 8:30am Monday to Friday. Whether you need advice from a doctor, or have an administrative request such as a sick note or test results, we will respond within 24-48 hours.

The NHS App is the simple and secure way to access your medical record using your smartphone, tablet, or desktop computer.

You can use the NHS App to:

book and manage appointments at your GP surgery
order repeat prescriptions
view your GP medical record securely, including
- test results
- vaccination history
- medications
- allergies
register to be an organ donor
choose how the NHS uses your data

New sick certificate

If you are ill for more than 7 days, you will need a doctor's certificate (also called a fit note or a med3 certificate).

If you have not seen the Doctor for the new medical problem you will need to request an appointment using the triage form for a new medical certificate. Please select the 'i need help with a medical issue' form.

If this is for an extension to a certificate you can complete the admin form on the 'contact us online' form.

Sick certificate extension

If you already have a sick certificate (also called a fit note or a med3 certificate) and need an extension, you can fill in the admin form online to request an extension from your GP.

This is the main 'contact us online' form on the home page but select the admin form please.

Ask a doctor a question

Ask a non-urgent question to one of our doctors and we will aim to respond in two working days.

Any health concern must be discussed in an appointment

Travel vaccinations

Travelling abroad and need vaccinations? Let us know about your trip at least 6 weeks before you go to arrange the vaccinations.

This will be passed to the Travel Nurse who will be in touch directly.

Request a letter

If you need a letter from your GP please complete the admin form section of the 'contact us online' portal on the home page of our website.

Please give as much detail as possible about why you need the letter, who it should be addressed to, and what needs to be included.

Change of address

If you have moved house, changed your telephone number, or changed your name please complete the admin form section of the 'contact us online' portal on the home page of our website.
