A photo of our care coordinator, Amanda. She is smiling at the camera.

Amanda provides information, support and guidance. She can help you feel supported and make sure you have access to local services that are right for you. Here are some of the areas she can help with:

  • Living with cancer
  • Supporting those who are housebound or frail
  • Arranging referrals for help at home
  • Links with community groups
  • Referrals for special dental care
  • Patient transport
  • Grief and bereavement
  • Loneliness and isolation
  • Living well with dementia
  • Carers support, including respite care
  • Housing issues
  • Support with addiction
  • Support for survivors of abuse

Finding the right support can be a difficult and daunting task. A care coordinator can help you find the right path to follow.

Ask reception to put you in touch with Amanda and she will get back to you.
